	<title>Condylus tertius: A case report study</title>

	<abstract>&lt;p&gt;&lt;strong&gt;Background and Aim:&lt;/strong&gt; The third occipital condyle (TOC) is an infrequent anatomical variation of the occipital bone that may imitate an occipital condyle (OC) fracture.&lt;/p&gt;

&lt;p&gt;&lt;strong&gt;Case Presentation&lt;/strong&gt;: Herein, we present an uncommon case of condylus tertius in a 33-year-old man with trauma with unilateral pain on motion (POM) and limitation of motion (LOM) of the head and neck without neurological deficits. The patient with a diagnosis of condylus tertius underwent conservative treatment with a Philadelphia collar and medical therapy. After two months of followup, he had a complete recovery of post-traumatic neck symptoms.&lt;/p&gt;

&lt;p&gt;&lt;strong&gt;Conclusion:&lt;/strong&gt; The TOC is an infrequent anatomical variation of the occipital bone that may imitate an OC fracture in trauma cases with good response to conservative treatment for post-traumatic symptoms and unknown effect of the occasional median joint of the TOC with C1 and or dens of axis as reinforcement of the craniovertebral region or hampering the motions of the head and neck&lt;/p&gt;</abstract>
